Terms and Conditions
These Terms and Conditions govern the use of services provided by FidelisX Diagnostics. By scheduling or receiving services, clients agree to the terms outlined below.
Service Agreement
FidelisX Diagnostics provides mobile diagnostic services including specimen collection, laboratory testing coordination, and related compliance documentation. Our services may include DNA testing, drug testing, specimen collection services, and other diagnostic screening services.
Clients agree to provide accurate information during scheduling and specimen collection. Failure to provide accurate identifying information may result in delays, cancellation of services, or invalid test results.
Payment Terms
All services must be paid in full prior to or at the time of specimen collection unless otherwise specified in a formal business agreement. FidelisX Diagnostics reserves the right to decline services if payment has not been received.
Pricing may vary depending on the type of service requested, travel requirements, laboratory processing fees, and additional testing needs.
Refund Policy
Due to the nature of laboratory testing and specimen collection services, certain services are non-refundable once testing procedures begin.
Refunds will not be issued once any of the following has occurred:
• Specimen collection has been completed
• Testing has been initiated or submitted to a laboratory
• Chain-of-custody documentation has been finalized
• A mobile technician has arrived at the scheduled service location
Once laboratory processing begins, refunds cannot be issued for completed tests, pending results, or dissatisfaction with the outcome of a test result.
Appointment Cancellations and Rescheduling
Appointments canceled at least twenty-four (24) hours prior to the scheduled time may be rescheduled without penalty. Cancellations made within 24 hours of the appointment may result in a cancellation fee.
Same-day cancellations or missed appointments may result in forfeiture of service fees or mobile travel charges.
Mobile Service Fees
Mobile service travel fees compensate technicians for travel time and on-site service preparation. These fees become non-refundable once a technician has been dispatched or has arrived at the appointment location.
Laboratory Processing
FidelisX Diagnostics partners with accredited laboratories for the analysis of specimens. Once a specimen is submitted to the laboratory, processing begins immediately. FidelisX Diagnostics does not control laboratory turnaround times, testing outcomes, or result determinations.
Clients acknowledge that laboratory testing is conducted independently by certified laboratories and that FidelisX Diagnostics serves as a specimen collection and coordination service.
Errors or Service Issues
If an error occurs due to FidelisX Diagnostics or an affiliated laboratory partner, reasonable efforts will be made to resolve the issue promptly. Corrective action may include recollection of a specimen or retesting when appropriate and feasible.
Limitation of Liability
FidelisX Diagnostics is not responsible for delays, errors, or interruptions caused by third-party laboratories, courier services, regulatory agencies, or other external entities involved in the testing process.
Clients agree that FidelisX Diagnostics' liability shall be limited to the cost of the services provided.
Compliance and Legal Use
Clients are responsible for understanding the purpose of their testing service, including whether results are intended for personal knowledge, legal use, employment verification, or compliance programs. Legal and DOT-regulated testing must follow strict chain-of-custody procedures.
Failure to follow required procedures may result in invalid results or the need for recollection.
Policy Updates
FidelisX Diagnostics reserves the right to update or modify these Terms and Conditions at any time. Updated policies will be posted or made available upon request.
